Job Duties:
The Acute Care at Home service primary focus is on maintaining older people at home in the event of a crisis or unexpected deterioration in health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and facilitate early discharge.
The post holder will function fully as a member of the Acute Care at Home Multi-disciplinary Team and will be required to participate in all multi-disciplinary meetings, share decision making, advise on specific dietetic requirements and complete patient held and shared files for all patients and carers in a person centered manner.

Qualifications / Experience:
3 years post-graduate experience in Dietetics at Band 6 level or equivalent.
Experience of working within community services.
